people who can no longer live at home without care and support and need
to live in a supported care situation.
Martindale offers quality
permanent residence for aged individuals who require a high level of
care and who have a current Aged Care Assessment.

The Martindale
Management team know from experience just how daunting it can be for
people not involved in the Aged Care industry, to look for just
that right place for your loved one to call home.
For this reason, we commissioned
a video/film to be made so that you and your loved one can view this
video at Martindale, or borrow and view in the privacy of your own home.
This video/film is not really about
Martindale, it's main focus is to concentrate on the admission process.
After being released late in
August 2003, it
will be available either on VCR tape to play in your video recorder, or
on VideoCD or DVD so you can play it in your standalone DVD player, or from
your computer if you have appropriate software installed.

1. Eligibility for Admission to Martindale Nursing Home
Prospective Residents must meet the
assessment criteria for high-level residential care. This assessment is
carried out by an appointed Aged Care Assessment Team (A.C.A.T.). Your local doctor
or Social Worker at your nearest hospital can refer a prospective
resident to the appropriate Aged Care Assessment Team. This health team
will assess the prospective Resident and complete the NH5 form and
decide whether the Resident is eligible for high level nursing care.
The form when completed, is sent to
the Commonwealth Department of Health & Ageing for processing and final
approval.

2. Prior to admission to Martindale

Prior to admission to the Martindale
Nursing Home, the Director of Nursing will interview prospective
Residents and their families to complete an application for admission
form.
Admission to the Martindale Nursing
Home can only be undertaken with a current NH5.

3. On admission to Martindale Nursing
Home, please bring
Completed NH5 Approval Form
Residents' Pension Entitlement and Medicare cards
S.A.
Ambulance (it is advisable to be a member of S.A. Ambulance to avoid
costly transport fees.
All
medications and any current prescriptions
All aids
or equipment currently required for the resident e.g. walking frame,
hearing aids, glasses etc.
If
transferring from a hospital, please bring a transfer/discharge letter

4. On Arrival
The Registered Nurse will admit the resident. Information supplied by
either the Doctor and/or resident and/or
carer/representative will be noted. The resident is encouraged to keep
his or her own General Practitioner, or Martindale Nursing Home will
assist in this matter if required. A list of visiting Medical
Practitioners is available on request.
A Nursing Care Plan for each residents' on-going care will be devised in
full consultation with the resident, relative or responsible carer and
the multi-disciplinary team in the nursing home.
A legal
document called the Residency Agreement will be given to the resident (or the
relative/responsible carer if they have a diagnosis of confusion), to
read and sign if they wish. The Agreement between the Proprietor and the
Resident protects both parties. It can be witnessed by a relative, staff
member or any other person of the residents' choice. The proprietor or
his/her appointed representative will also sign all documents. A
photocopy of the Agreement will be made available to the resident, if
required.

5. Security of Tenure
Accommodation consists of private,
share, 3 and 4 bed wards. Every effort will be made to settle each
resident into their new life with the nursing home. However, there may
be some circumstances in which a resident may be asked to leave the nursing
home.
Examples of such circumstances are:
the
resident no longer needs 24 hour nursing home care
the
resident becomes a fire or safety hazard to other residents, staff or
the nursing home
the
well being of the residents is affected by his/her being in the
nursing home
Non-payment of fees (following discussion with the Director of Nursing
as to the reason for non-payment)
Any resident who, for any of the
above stated reasons, needs to leave the nursing home will be assisted
to find new accommodation by the Director of Nursing
A resident is rarely asked to change
rooms, but may be asked to do so if there is a change in the residents'
condition or for the smooth running of the nursing home. Consultation
will be held in these cases with the resident, relative or responsible
carer and staff and options will be discussed.
If a resident requests a room change,
every effort will be to assist with such a request, providing all
residents being moved are given the opportunity for the same
consultative processes.
Residents who share rooms are
selected as carefully as possible in relation to compatibility, comfort,
health and intellectual needs of the nursing home.
